Transition of Care
Simplifying safe patient transfers
MetaVision Transition of Care delivers the tools you need to ensure safe, efficient handovers across units and care settings.

-
TOC is purpose-built to streamline the handover of treatment plans and ongoing care between units or care settings. It ensures the continuous flow of critical information, reducing delays and minimizing the risk of miscommunication that can compromise safety or increase costs.
-
With a dedicated interface, clinicians can easily import and export data about ongoing and planned treatments between MetaVision and external systems. This ensures continuity of care at every step—improving patient safety and supporting better clinical outcomes.

Logging and reporting
Tracking and reporting of all data imported and exported from and to the system, allow full traceability of treatment activities and decisions.
We decided to stay with MetaVision mostly for two reasons. One is that hospital-wide systems are not dedicated to complex environments like the ICU. And the second one is interoperability, which is the future of all ICT in healthcare. A system should interface with every other system around it, and MetaVision is very capable in doing that.”

What is required from the third-party to integrate the TOC feature?
FHIR R4 RESTful APIs aligned with the configurable and fully documented iMDsoft profile.
-
My third-party vendor does not have support for FHIR, can I still use the TOC solution?
Yes, If the data can be exchanged with the third-party system, integration can be possible. However, it requires the development of third-party convertors. In case an integration engine is used (e.g., Mulesoft or ensemble) it may be possible to configure such a convertor onsite (managed by the hospital IT).
-
Can I integrate the TOC feature with two different systems?
Yes, the TOC feature allows the customization of different buttons linked to two different systems. However, multiple servers may be needed to facilitate the communication.
-
Is integration possible with third part systems that do not have a fully synced drug/task/process library with MetaVision?
Yes, however medications/tasks/processes which cannot be translated to a corresponding item in MetaVision using a synced code library will be imported as a "Generic" item which will need to be adjusted in MV once the import is done or in the third party once the export is done.
